About the Lymphatic System

The lymphatic system includes lymph, lymphatic vessels, and lymphoid organs and tissues. It plays an important role in the body’s immune defenses, fluid circulation, and maintaining the right balance of fluid within the tissues.

Lymph is a fluid formed from excess fluid that collects in the spaces between cells. It travels through the lymphatic vessels, is filtered by the lymph nodes, and eventually returns to the bloodstream.

The main functions of the lymphatic system include:

  • Collecting excess fluid from the tissues and returning it to the bloodstream.
  • Filtering lymph and removing microorganisms or foreign substances.
  • Producing, developing, and activating lymphocytes.
  • Protecting the body against infections.
  • Absorbing and transporting fats from the digestive system.

The lymphatic system works closely with the hematopoietic system and plays an essential role in normal immune function.


Main Components of the Lymphatic System

Primary Lymphoid Organs

Bone Marrow
Bone marrow is the tissue where blood cells are produced. B lymphocytes develop and mature in the bone marrow, while immature T lymphocytes travel to the thymus to continue their development.

Thymus
The thymus is the organ where T lymphocytes mature and develop their specialized functions in the immune response.


Secondary Lymphoid Organs

Spleen
The spleen is the largest lymphoid organ in the body and is located in the upper left part of the abdomen. It filters the blood, removes old or damaged cells, and helps protect the body against infections.

Lymph Nodes
Lymph nodes are small structures located along the lymphatic vessels. They filter lymph and provide an environment where lymphocytes can become active and take part in the immune response.

Tonsils
The tonsils are collections of lymphoid tissue at the back of the throat. They help protect the body against microorganisms that enter through the mouth and nose.

Mucosa-Associated Lymphoid Tissue (MALT)
This tissue is found in the lining of the digestive, respiratory, and urogenital systems. It includes structures such as Peyer’s patches and the appendix.

Lymph and Lymphatic Vessels
Lymphatic vessels collect excess fluid from the tissues and transport it as lymph. As it circulates, lymph passes through the lymph nodes, where it is filtered and monitored by immune cells. It eventually returns to the bloodstream through the main lymphatic ducts.


Common Conditions Affecting the Lymphatic System


Enlarged Spleen (Splenomegaly)
A condition in which the spleen increases in size. It may occur with hematologic diseases, infections, or liver disorders.

Hypersplenism
A condition in which the spleen becomes overactive, resulting in lower numbers of red blood cells, white blood cells, or platelets in the blood.

Hyposplenism
A condition characterized by reduced spleen function, which limits its ability to filter the blood and protect the body against infections.

The spread of cancer cells from other organs to the lymph nodes. Their presence helps doctors assess how far the disease has spread.

Symptoms of Lymphatic Disorders

Common symptoms include:

  • Enlarged lymph nodes in the neck, armpits, or groin.
  • A persistent fever or night sweats.
  • Unexplained weight loss.
  • Severe tiredness and weakness.
  • Frequent or prolonged infections.
  • A feeling of heaviness, fullness, or swelling in the abdomen.
  • Persistent itching of the skin.

Diagnosing Lymphatic Disorders

A diagnostic assessment may include:

  • A review of your medical history and a clinical examination of the lymph nodes, spleen, and liver.
  • A complete blood count and microscopic examination of a blood sample.
  • Blood chemistry and other laboratory tests, when appropriate.
  • Imaging tests, such as ultrasound, CT, or PET-CT.
  • A lymph node biopsy when a tissue examination is needed.
  • Bone marrow aspiration and biopsy when bone marrow involvement is suspected.
  • Specialized tests, including immunophenotyping and cytogenetic or molecular analyses.

The choice of tests depends on your symptoms, clinical findings, and the suspected diagnosis.

  • Enlarged lymph nodes that persist for more than two to three weeks.
  • A fever or night sweats without an obvious cause.
